Referral Forms (Funded 1-1 Intervention)

We offer referral pathways for funded 1-1 intervention delivered through the charity.

To ensure we provide the most appropriate support, we speak directly with the relevant professional before issuing referral forms. This allows us to understand your needs and ensure the referral process is safe, appropriate, and tailored to the pupil’s circumstances.

• Monday–Thursday: Working in schools and with our partners

• Friday: At the Hub – MHS Homes, Broadside, Chatham, ME4 4LL 9:00 AM – 3:00 PM

• Based in Medway, Kent, with team members located across the UK, we deliver our services nationwide with a strong local and regional understanding.
